Application For Enrolment

PLEASE COMPLETE ONE (1) APPLICATION PER CHILD

This application will take 30 minutes to complete. Please have your child’s birth certificate, immunisation statement from Medicare, (whether you are immunised or not) saved on your computer as a jpeg or pdf ready to upload. Including any relevant school reports or medical diagnoses.

Checklist

Your application is eligible for assessment when you have:

  • Included the Application Fee – a non-refundable administration charge applies for each
    application for enrolment. Please refer to current fee schedule for details.
    EFT: Golden Hill Steiner School, BSB – 086 595, A/C – 174989795 – please reference with
    surname.
  • Attached a copy of your child’s birth certificate.
  • Attached a copy of your child’s two (2) most recent school reports. (If your child is
    currently home-schooled, please attached sample copies of work from the past 6
    months, including maths samples).
  • Attached a copy of extra needs assessments/reports (e.g. psychological assessment,
    specialist report, learning support program, behaviour management plan etc.).
  • Signed the last two pages of the Enrolment Application form.
  • Attached a copy of your child’s AIR (Immunisation History Statement) – This is a WA
    Government Requirement and is required regardless of your child’s immunisation status.
    The AIR must be dated within two months of the Enrolment Application.

* All schools in Australia are required to collect some additional background information on students as part of national reporting arrangements. This information is collected by schools in a uniform way across the country, in compliance with the Australian Education Act 2013 s.77.

The information collected will not be used to identify individual students. It will be used to better understand how background factors may affect student educational levels, and help in developing policies to better assist those children not meeting expected standards.
